110-300-0165(2)(d) Licensor observed sleeping mats stored in white plastic trash bags on the floor under the fireplace accessible to children. 110-300-0165(2)(f) Licensor observed the sliding glass door did not have stickers or artwork at child's eye level to prevent children from walking through the glass. 110-300-0165(2)(g) Licensor observed a play pen next to a window 110-300-0165(3)(d) Licensor observed washer and dryer accessible to children with no safty lock 110-300-0165(3)(e) Licensor observed tripping hazards in the space accessible to children. The bathroom transition piece is loose and several carpet overlapping inside and out side of the house. 110-300-0165(4)(c) Licensor observed bathroom light fixtures and entry was ceiling fan did not have shatter proof coverings over light bulbs. 110-300-0165(4)(f) (A) Licensor observed front outside stayway to the door had no railing on one side with a 26.5 inch drop to concrete. Otherside of stairway had slat opening of 6 inches on the railing. Licensor observed the outside stairs to the back playground area had slat opening of 4.75 inches and no gate at top or bottom of the stairs to prevent children accessing the stairs when not in use. 110-300-0165(5)(a) Licensor observed electrical outlets did not have automatic shutters or blank plates in the area accessible to children. 110-300-0165(5)(b) Licensor observed electrical outet in bathroom was not a GFCI.
